Message for Medicaid Members: Have you moved in the past three years? Has your address or contact information changed? Please make sure [MEDICAID/FAMIS] has your current mobile phone number, email, and mailing address so our records are up to date. It’s important to make sure we can reach you with information about changes to your health insurance. There may be steps you need to take to keep your coverage. How do I get started?

When making your first appointment with us you will be asked for your full name and date of birth. It will help us find you in the Medicaid system and verify your benefits. Do not forget to bring your Medicaid card and photo ID with you for your appointment as well as there 3 form filled: Medical History Form, Clinic Policy Form and Consent Form (or Consent Form for Minor).

For covered services, the ARDC team will take care of submitting claims to Medicaid after your visit and there will be no out of pocket cost for you. The services below are covered by most Medicaid plans:

  • Annual exams 

  • Emergency consultations

  • Routing bi-annual cleaning

  • Fillings

  • Extractions

  • Root canals

  • Crowns (with prior authorization)

For services not covered by Medicaid, no surprises, we will provide you with a quote prior to making your appointment. You will be asked to pay for the needed services in full prior to your visit.  

Patients are responsible for maintaining their Medicaid benefits. If you appear ineligible the day of your appointment, you will be asked to pay a copay for your visit (see “I am uninsured” section). ARDC patients are welcome to reschedule their appointment up to 24-hr before the appointment. Appointments canceled less than 24-hr before the visit and appointments for which patients do not show up (or arrive more than 10 min late) count as missed appointments. After two missed appointments, patients will be dismissed from the ARDC for one year (see FAQ for more information on compliance and dismissals).
